Visitor COVID-19 Screening Form

All visitors to the Wells College campus are expected to abide by the same precautions that those in our community have committed to upholding — to protect the safety of all in our community.

VISITOR GUIDELINES

  • Although campus is not closed to the public, visitors should make an appointment in advance. Faculty and staff are required to keep a visitor log, in the event such information is needed for contact tracing purposes.
  • We recommend limiting the size of your party to three or fewer individuals.
  • Fully vaccinated* individuals are not required to wear masks. Non-vaccinated individuals are required to wear masks and practice social distancing at all times.
  • All visitors to campus, including family members, must fill out the Visitor Screening Form (see below) prior to their arrival.

If you have any questions about your upcoming visit to campus, please contact the Office of Campus Safety at 315.364.3229. The office is staffed 24 hours a day, 7 days a week.

Visitor Screening Form

This form must be completed by any visitor to the Wells College campus (except for delivery personnel, e.g., USPS, UPS, FedEx, Staples, etc.). Wells College students and employees should use this form instead.

VACCINATION STATUS

* According to the CDC, you are fully vaccinated two weeks after your second dose in a 2-dose series (such as the Pfizer or Moderna vaccines) or two weeks after a single-dose vaccine, such as Johnson & Johnson’s Janssen vaccine.

SYMPTOMS

*COVID-19 symptoms include: cough, shortness of breath or difficulty breathing, fever, chills, muscle pain, sore throat, new loss of taste or smell, and (less commonly) nausea, vomiting or diarrhea, fatigue, headache, congestion or runny nose. Fever is considered to be over 100 F / 38 C. NOTE: If you are experiencing any of these symptoms, even if they are mild, answer Yes.

RECENT CONTACT

** "Close contact" is defined as being closer than 6 feet for more than 10 minutes.
